New Patient Information

Thank you for selecting our office for your mental health care.  Before we can schedule an appointment, we have 5 required forms for you to complete (select Required Forms on the left).   After your forms are completed, you can mail, fax, email or hand carry them to our office.   Because we schedule 2 to 3 - 60 min. appointments for the initial evaluation, we require a $100 deposit check or credit card to secure your appointment times.  If you are mailing your forms, please enclose a check for $100.  If you are faxing, emailing or bringing the forms to our office, we request you provide us with a credit card for the deposit.  The deposit payment will be held in our office until your first appointment at which time it will be applied towards the fee of your first visit.  If you miss your initial appointment and do not give us 24 hours cancellation notice, we will use your deposit for the missed appointment fee and cancel all other scheduled appointments.  (see missed appointment policy and office policy)
