Minor New Patient Evaluation
(under 18 years old)
(2 hours) $510/hr
For minor patients under 18 years of age, the intial evaluation will be split into two parts. The first hour will be scheduled with the parents/guardian of the patient. The second hour will be scheduled for the patient.

For patients over 18 years of age there will be only one first initial evaluation.

Miscellaneous Fees
Letters and/or forms requested by the patient may be billed according to the complexity of the letter or form, and determined by the length of time taken to complete. Extended patient telephone calls and calls to other professionals (attorneys, therapists, school personnel, etc.)

Any session that is cancelled or missed with less than 24 hour notice will be billed at the same rate as the scheduled length of time of your visit.

How to submit receipts to insurance
After an appointment has been paid in full an itemized receipt (also known as 'Superbill') will be available on your patient portal.
This itemized receipt includes all the necessary information your insurance requires in order to process a claim. It includes the following information: Patient Name, Date of Birth, Date of Service, Procedure Code (as known as 'Service Code' or 'CPT Code'), Service Provider, Provider TAX ID, Provider NPI, Cost of Service, and Payment of Service.
To download or print your itemized receipt please follow these directions:
- Login to patient portal here. If you need any assistance with your patient portal login, please contact our front office team.
- Once you are logged into your patient portal, select the 'Billing' section on the left hand side bar.
- Then select the 'Ledger' tab on the top left.
- Under 'Patient Financial Ledger' you will see all claims for services you have paid in full. (Note: if you do not see a particular date of service please contact our front office team for assistance.
- Within the 'Ledger' page, you can search or select the claims or dates of service that you are seeking by clicking the check box next to each (or unselecting the dates of service you do not wish to print/download).
- Once you have the claims or dates of service select make sure to select the small (+) symbol at the top of the section; this will expand the details of each claim to include all the necessary information you will need.
- Once you have selected everything that you need you can select 'Print' and from the new window download or print your receipt.
From here you will need to navigate your own insurance account to upload/submit your claim with the information provided on your itemized receipt.
